We are looking for a bilingual Nurse Innovator to help us lead our practice as we serve the needs of members of the Culinary Health Fund who have one or more chronic illnesses.
Essential Duties and Responsibilities
The Nurse Innovator is a key leader who partners with the Medical Director to run the practice. Specific responsibilities include:
Train, supervise, and lead our unique care team of Health Coaches, Social Workers, and Administrative Staff to support patients? clinical and social needs, both at the practice and in group meetings in the community.
Develop strong team environment through ongoing training, performance feedback, goal setting, and problem solving.
Cultivate a culture of innovation and creativity in service of better patient care.
Actively manage practice activity to ensure smooth patient flow, timely care, and outstanding service to patients.
Ensure that the physical practice is properly equipped with the necessary clinical tools, supplies, and resources.
Support health coaches in helping patients navigate the health care system when secondary or tertiary care is needed.
Triage patients, and provide direct clinical care.
Maintain necessary regulatory & clinical safety requirements
Licensed Registered Nurse, who has or can obtain a Nevada state license.
Fluency in English and Spanish required
At least five years of nursing / professional experience
Familiarity with the Las Vegas community is a plus.
Comfort working with a chronically ill population
Excited to create a new program from the ground up; idealistic, thoughtful, articulate, hard working, creative, flexible, able to deal with uncertainty and adapt to new technologies
