Grameen PrimaCare and Iora Health have partnered to bring innovative primary care to low-income female entrepreneurs and their families in Jackson Heights, Queens. Iora Health is looking to hire a bilingual, full-time Registered Nurse to play the role of Nurse Innovator at this new practice.
Grameen PrimaCare is a non-profit organization whose mission is to provide high-quality, affordable health care and education to underserved communities. Grameen PrimaCare is partnering with Iora Health at this practice to serve women receiving micro-loans from Grameen America.

Essential Duties and Responsibilities:
The Nurse Innovator is a key leader who partners with the Medical Director to run the practice. Specific responsibilities include:
Train, supervise, and lead our unique care team of Health Coaches, Social Workers, and Administrative Staff to support patients? clinical and social needs, both at the practice and in group meetings in the community.
Develop strong team environment through ongoing training, performance feedback, goal setting, and problem resolution.
Cultivate a culture of innovation and creativity in service of better patient care.
Actively manage practice activity to ensure smooth patient flow, timely care, and outstanding service to patients.
Ensure that the physical practice is properly equipped with the necessary clinical tools, supplies, and resources.
Support health coaches in helping patients navigate the health care system when secondary or tertiary care is needed.
Triage patients, and provide direct clinical care.
Licensed Registered Nurse, who has or can obtain a New York state license.
Fluent in English and Spanish.
At least five years of nursing / professional experience is preferred.
Pediatric and women?s health experience is preferred.
Familiarity with the Jackson Heights community is a plus.
Customer service experience and experience facilitating group learning a plus.
Excited to create a new program from the ground up; idealistic, thoughtful, articulate, hard working, creative, flexible, and able to deal with uncertainty.
