Habit OPCO is one of the largest accredited healthcare organizations in New England providing multi-disciplinary treatment services on an outpatient basis to individuals addicted to drugs enabling them to function normally in society.
Habit OPCO commenced operations in Boston, Massachusetts in January 1986. Today we are providing treatment services at 21 sited and 2 mobile sites throughout Massachusetts, New Hampshire, Vermont, Pennsylvania, and New Jersey.
Individuals who seek treatment at Habit OPCO are everyday people struggling to overcome addictions. They have families, friends, jobs, hobbies and dreams. They come from all walks of life and age groups. All of the patients treated at Habit OPCO come here voluntarily, out of a desire to receive treatment to improve their lives.
Habit OPCO provides rehabilitation treatment services on an outpatient basis for patients with drug addictions, enabling them to function normally in society. Our highly-trained professionals, many of whom are Masters-prepared, provide individual, group and family counseling focused on addiction, recovery, and related personal, family, and vocational issues.

Our philosophy is based on the fact that methadone, combined with life skills counseling, has been shown to be the best treatment available for opiate-dependent individuals. We understand that reducing the stigma associated with narcotic addiction and treatment will always be a challenge, but it is a process to which we are firmly committed.
As a Care Coordinator, you will become an integral part of Habit OPCO?s multidisciplinary treatment team, helping patients in accessing medical, clinical, vocational, and other rehabilitative services as indicated in the biopsychosocial assessment.

As a Care Coordinator you will act as liasion between patients and collateral agencies providing Spoke service. The Care Coordinator assists patients in accessing medical, clinical, vocational, and other rehabilitative services.
Your specific duties as a Care Coordiantor are, but not limited to:
Facilitates referrals to Spoke agencies based on assessments of need.
Documents all contact with patients and provider using appropriate service in the eectronic health record
Obtains and provides intake referral informtationto patients in a timely manner
Ensures that a minimum of one health home function and one face-to-face clinical contact is provided for each Hub patient on a monthly basis.
Perform monthly audits of coordinating activities to supervisor and MDT.

As a Care Coorindator, you must have the ability to cultivate positive and productive relationships with patients while always maintaining appropriate professional boundaries. You must also be self-motivated; possess strong time-management and organizational skills; and have the ability to manage a high volume of client interactions.
It will also be critical in the role of a Care Coordinator that you have strong verbal and written communication skills and exceptional interpersonal instincts that will allow you to function effectively as part of a team in a fast-paced, intense work environment. We?re looking for a problem-solver with a positive attitude, someone who is open to receiving feedback and incorporating new treatment approaches; who enjoys leading group counseling sessions; and who has demonstrated success working with diverse populations.
Specific qualifications for the Care Coordinator position include:
Bachelor?s degree in a related field.
Good Assessment, writing and interpersonal skills
Experience working in a substance abuse setting a Plus
Familiarity with Clinic Management Software a Plus
Basic Computer Skills (MS Office) & the Desire/Ability to Learn New Software
