Job Summary:
In alignment with company mission, vision and values, and in accordance with applicable state regulations, the Care Manager coordinates, implements and manages service delivery to provide quality care to clients and their families. Duties include:
Performs comprehensive psychosocial assessments to determine potential client service needs
Develops, implements and revises client care plans and budgets.
Coordinates care with Staffing Coordinators, staffing support, Caregivers, and other care providers.
Interviews, selects and provides ongoing supervision of in-home Caregivers to ensure individual client needs are met; initiates corrective actions where needed.
Contributes to extending client expected length of stay (ELOS) through the delivery of exceptional care management services.
Documents and updates activity in multiple systems to ensure compliance with state regulation and company protocols.
Maintains individual productivity/gross profit targets based on company guidelines.
Anticipates client/caregiver issues in order to proactively address and maintain quality care within client home.
Other duties as assigned.
Minimum Experience / Credentials / Education:
Two years experience in geriatrics, care management or related disciplines required.
Knowledge of MA home care regulations preferred.
Ability to interface effectively and professionally with client support network, including family, physicians, attorneys, etc.
PC literate.
Frequent local travel by personal automobile to client homes required.
