Perform duties of a Consultant Pharmacist in long-term care setting.
Education & Qualifications:
Current registration and licensure as a Pharmacist in the state of Virginia.
High level clinical evaluation and decision making skills. Good organizational skills, public speaking, ability to teach, excellent verbal and written communication skills, good social skills, ability to work under pressure and meet deadlines, self motivated, and problem solving skills. Familiarity with state and federal pharmacy and long-term care regulations.
Flexible, customer service oriented, and cooperative, personable, leader, dependable, honest, and respectful.
Perform medication regimen reviews in long term care and assisted living/independent living settings as required by federal and state regulations
Perform routine medication storage inspections and assist with the appropriate disposal of controlled substances.
Provide operational and clinical education to facility staff and prescribers
Observes medication passes of facility nurses
Reinforce facility compliance to pharmacy policies and procedures, as well as state and federal regulations.
Provides reporting, documentation (trend reports and summaries) and any other functions as required by federal or state nursing facility regulations
Participate in facility QA Programs and Interdisciplinary Team meetings
Maintain thorough and appropriate documentation in the client record of all clinical activities and communicates with client-DON?s Administrators, physicians, and other healthcare professionals in a format convenient to client at a time convenient for client
Maintain clinical skills and pharmacy knowledge by reviewing literature and attending educational seminars
Supervises activity of Pharmacist Technician(s) or Pharmacy Student(s) assigned to any of the Consulting Pharmacist?s Clients
Track and complete expense reports
Exhibits ability to communicate verbally and in writing with good English language skills
Adequate vision and hearing to perform job requirements with or without corrective appliances
Ability to bend and lift 30 pounds
Subject to random drug screening
