Cogent HMG is the industry leader in developing and managing hospital medicine programs nationwide. We align with dozens of hospitals to integrate hospitalist programs that best serve patient care needs and achieve goals of quality, safety, efficiency and service. We are currently seeking an experienced Director of Value, Performance and Innovation at our corporate office in Brentwood, TN. This corporate level position establishes, maintains and champions the company?s quality measurement and improvement systems. He/she leads the negotiations with prospects and clients to configure quality measures and metrics to the local provider needs, yet preserves an overarching standard that defines the Cogent HMG Quality Enhancement Program. He/she collaborates with business unit and field personnel to install and maintain the Cogent HMG quality measurement program. He/she ensures that the program is aligned with national quality and payer organizations that define and pay for performance. He/she develops and maintains business intelligence and analytics that provide leaders at all levels with insight into opportunities for improvement and value creation. He/she has a prominent role in developing clinical and operational care delivery models and solutions. In addition, he/she oversees the Homecoming Call Program.
Key Responsibilities
Evolve and manage the company?s Quality Enhancement Program (QEP) and other quality management initiatives/systems
Negotiate QEP measures and metrics with each client
Train corporate and field personnel on QEP implementation, tracking and improvement
Ensure that corporate systems facilitate warehousing, reporting and trending of QEP performance
Design and administer Nursing Medical Management Survey
Support the administration of the PQRS Program
Facilitate regular assessment and definition of the Company?s current QEP program and data collection and reporting processes
Evolve business intelligence and analytical capabilities that enable us to understand and report relative performance, best practices, underlying performance drivers and opportunities for improvement
Support local program management in the preparation for interim reporting and Joint Operating Committee (JOC) meetings with clients
Assist Program Managers with review of program JOC reports for data integrity, accuracy and meaningfulness
Facilitate ongoing innovation of effective presentation of outcomes, insights and action plans
Identify performance outliers and collaborate with corporate and local personnel to address
Partner with Information Technology (IT) to Evolve and Maintain the Data Warehouse
Maintain enhancement list
Collaborate with IT to produce data warehouse and reporting enhancements
Test new releases
Provide corporate and field education regarding enhancements and new reports
Oversee Homecoming Call Program and Personnel
Evolve and maintain Homecoming Call Program to ensure contractual, clinical and operational outcomes achievement
Supervise and mentor Homecoming Manager
Review daily Homecoming productivity reports
Review and approve monthly Homecoming incentive reports
Provide training and ongoing support of local program management to optimize performance and identify trends and needs
Collaborate with Clinical Management, Medical Management, Innovation and IT to develop new initiatives and solutions (on ad hoc basis)
Education, Training and Experience
8+ years healthcare experience in the areas of healthcare quality and performance measurement, business intelligence, and clinical product development.
High degree of acumen in analyzing data using industry standard tools, including excel and SQL.
BSN required, advanced practice degree desirable.
Experience leading teams and influencing leaders. Excellent interpersonal and communication skills. Strong attention to detail and service orientation.
