Due to the success of our Brooklyn practice offered through Freelancers Medical, we are looking to hire more full-time Health Coaches.
Freelancers Insurance Company (FIC), Collective Primary Care (CPC), and Iora Health are partnering to revolutionize primary care through our unique care model. This model of health care changes everything. Patients will enjoy easy appointment scheduling, access to their doctor and health coach by phone and email, and free wellness groups (such as yoga and meditation). Care teams will enjoy the benefits of smaller panel sizes, closer relationships with patients, no billing or coding, and the opportunity to lead systemic change in health care delivery while working with a true team. At the heart of our new staffing model is a team of health coaches, who work with patients to coordinate services, unlock personal motivation, and make healthy lifestyle changes. This model of care delivery dramatically improves the patient care experience as well as the quality of clinical outcomes.

Iora Health is a rapidly growing primary care management services organization that is opening such innovative primary care practices nationwide in an effort to change care delivery as we know it today.
Essential Duties And Responsibilities:
Reporting to the Nurse Innovator, the Health Coach serves as a key member of the care team and plays a critical role in helping patients improve their health. They engage and encourage patients to take an active role in their health by providing the tools necessary to make healthy lifestyle choices and adopt lifelong healthy behaviors. Our Health Coaches help patients unlock personal motivation to make lasting changes and simultaneously serve as the patients? primary conduit to the practice as they help coordinate necessary services both within and outside of the practice.
In addition to their active clinical involvement, Health Coaches rotate through the roles of greeting patients to the practice and providing patient support wherever needed. As a result, they have the opportunity to influence the patient experience in its entirety, from the moment patients set foot in the practice to the time they leave and well beyond. We believe understanding and fostering the continuity of the relationship helps build the strength of the collaborative culture our practice embraces.
Specific responsibilities include, but are not limited to the following:
Collaborating with the practice team to help actively engage 200 patients in their care and facilitate the implementation of their customized care plans
Communicating with patients in person, in groups, by telephone, and via electronic means
Engaging with patients one-on-one and in group settings
Implementing a range of interventions/skills including counseling, motivational interviewing, coaching, education, referral and advocacy to gain access to services and resources for patients
Providing customized, evidence-based patient education in a variety of areas including but not limited to weight management, tobacco cessation, and chronic disease self-management
Communicating care plans and pertinent information with the internal practice team as well as with other parties participating in the care of the patient
Assisting with the coordination of care across the care continuum (including home care, outpatient care, ER care, and hospital care)
Performing all necessary administrative intake functions
Must have a service mindset or experience working in a service environment
Must serve as a role model and be able to ?walk the wellness walk,? not simply ?talk the wellness talk?
Must embrace the NYC freelancers community
Must be open-minded, non-judgmental and compassionate
Must demonstrate excellent interpersonal communication skills including exceptional listening skills
Must have the ability to work well both as a member of a team and independently
Must have exceptional organizational, problem-solving and time management skills
Must demonstrate intellectual independence and willingness to learn new skills
Must be flexible and comfortable with ambiguity and change
Must have experience leading or a desire to learn to lead group classes such as yoga, zumba, nutrition, cooking, athletic training, stress management, meditation, relaxation, pregnancy and child birth, etc.
