Health Educator - CDE - Bilingual CCS Medical Holdings, Inc.
Overview:
To provide health education to patients and staff under the direction of the Director, Clinical Services
Responsibilities:
Provides telephonic health education and information to patients that are referred to the Clinical Services department through the PIMS system
Provides assessment, education, instruction, coaching, goal setting, evaluation, data collection and documentation of patients.
Assists the Learning and Development department , as well as, the Sales department in the creation, implementation and evaluation of education programs and training materials for CCS Medical employees
Provides a clinical evaluation for new products and services for the company
Assists in the development and maintenance of health educational materials in a variety of formats
Completes medical chart reviews and audits for the Compliance department, as directed
Other Duties:
Assists Human Resources in the assessment, planning, implementation and evaluation of employee wellness and health promotion for the organization, as needed
Provides one-on-one wellness instruction for CCS Medical employees, as needed
Represents and promotes CCS Medical at trade shows, conferences, community events, and community programs, as needed
Attends annual conferences for continued education, as directed
Maintains and promotes patient confidentiality
Maintains regular and consistent attendance
Maintains open lines of communication with Administration, department management and other departmental personnel
Follows all Medicare, Medicaid and Private Insurance regulations and requirements
Actively participates in Continuous Quality Improvement
Demonstrates thoroughness with all work related activities. Strives to continually improve quality and productivity results displayed by Department Personnel on an ongoing basis
Assumes other duties and responsibilities as assigned by manager/director
Maintains scheduling flexibility to ensure availability to meet the needs of the department for all hours of operation
Abides by all policies, procedures and protocols set forth in the departmental, CCS Medical Personnel and Administrative Policy Manuals
Qualifications:
Minimum Bachelor?s Degree from an accredited college or university and two years of diabetes education experience and/or training, or equivalent combination of education and experience.
Registered Nurse (RN) or Registered Dietitian (RD)/Licensed Dietician (LD), required
Diabetes Educator certification (CDE) by National Certification Board of Diabetes Educators and Insulin Pump certifications are required
Bilingual Spanish required
Experience with wound care, urology and ostomy patients, desired
Experience using motivational interviewing techniques and behavioral change principals with patients.
Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or government regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations
Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables
Proficient with various Microsoft Office software such as Word, Excel and PowerPoint
Advanced knowledge of processing methodologies and medical terminology
Knowledge of CCS Medical product line(s), general coverage, clinical criteria, regulatory and compliance standards
Excellent planning, organization, and execution skills
Facilitation/presentation skills
