Home Program Nurse Manager - RN

Supervisory Responsibilities:
The Nurse Manager is responsible for the overall direction, coordination, and evaluation of the clinic's programs. Carries out supervisory responsibilities in accordance with the Liberty Dialysis policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
Role and Responsibilities:
Reviews patient care policies and procedures for maintenance of highest standards of care and most current techniques.
Communicates patient care policy and procedure changes with home program staff
Evaluates the quality of patient care provided by home program staff
Promotes open communication and interaction between nursing and other disciplines to facilitate quality patient care.
Reviews and understands operational policies and procedures to ensure compliance with various local, state, and federal regulations.
Supervises staff in managing supply costs for patients and for the facility
Ensures standardization of clinical, training and billing practices
Facilitates QAPI initiatives for the home program
Promotes PD awareness within HD facilities across the market.
Coordinates pre-dialysis treatment options education program offered to all referring physicians
Acts as a liaison between Administration and other department heads and staff.
Ensures facility is inspection ready.
Routinely holds staff meetings to share information and provide clinical education.
Ensures timely completion of care planning process for the facility
Acts as a resource for clinical and operational problem solving for all staff in the Home Dialysis Program
Maximizes access to resources within Liberty and with outside Vendors or other resources
Develops a training plan for new employees
Attends divisional and market management meetings as needed
Determines staffing needs. Interviews and hires home program staff in partnership with the Director of Operations.
Completes Human Resource functions related to payroll, personnel files, transfers, staff evaluations, etc.
Performs disciplinary actions as needed.
Promotes Liberty team approach within the home program.
Develops educational tools for patients and staff as needed.
Reviews and assists staff with Clinical Ladder action plans and progress as needed.
Schedules regular meetings with Home Dialysis Medical Director to review problems, quality improvement issues, policies and procedures, and provides a program update.
Promotes nursing education and advancement.
Home Program Nurse Manager oversee's multiple small home programs in the Salt Lake City area. Must be willing to travel between the clinics on a routine basis.
Required Experience
At least two years of experience as a Registered Nurse within Dialysis.
At least 6 months of experience in Peritoneal Dialysis.
Experience with home hemo dialysis a plus.
Must be able to lift a minimum of 35 lbs.
Leadership experience is highly desired.
Experience in a fast paced critical care setting strongly preferred.
Current RN license in good standing.
