Job Description
Occupational Summary:
Supervise daily functions of the unit to ensure effective delivery of home patient care and compliance with clinical policies and procedures.
Duties:
Plan and organize work for efficient completion of daily operations of the clinical unit playing an active role in the daily operations.
Responsible for day-to-day nursing services; provide oversight and direction to all direct care staff that provide dialysis and nursing care in the facility, ensuring that physician orders are carried out.
Provide on-call services to the patient as necessary and as scheduled by the Nurse Manager.
Meet and evaluate prospective new home patients, explain modality options and home dialysis requirements.
Be a liaison between the home patient, supply companies, transplant team, and physician to ensure continuity of care.
Oversee the completion and documentation of dialysis equipment.
Oversee and schedule time for care plan meetings and charting to maintain complete and accurate records.
Develop, teach, interact, and evaluate delivery of home patient care with patients and their families to promote participation in patient care.
Maintain required clinic records and reports for staff and patients, (i.e. hepatitis tracking, patient and employee labs etc.).
Reinforce unit compliance with established policies, procedures, objectives, QAPI, Risk management, and infection control.
Maintain communication with physicians, ancillary services, and other healthcare providers
Complete daily computer entry and reporting requirements.
Complete and attend required educational programs.
Education/Experience:
Graduated from an accredited Registered Nurse program with current North Carolina license
Dialysis experience preferred
CNN certification preferred
BSN preferred
BCLS and ACLS certification preferred
Skills:
Leadership ability and organizational skills
Computer software knowledge
Ability to communicate effectively
Ability to understand and implement government standards
Physical Job Requirements:
Frequent long periods of standing, bending, stooping, walking, sitting
The ability to lift up to 30 lbs
