Under direct supervision, abstracts and codes patient records in compliance with coding, billing and data collection guidelines of the organization. Position involves physician based coding for primary care as well as a variety of specialties.
Accurately abstracts information from the medial records into the appropriate coding systems, ensuring compliance with established guidelines.
Determines the most appropriate diagnosis after a thorough review of the medical records. Works closely with practice staff with regards to coding and assignment of ICD-9, CPT and HCPCS of medical records using ICD-9-CM and CPT-4 coding rules and guidelines. Ensures thorough and compliant coding to support patient records and submission of billing for payment.
Accurately sequences diagnostic and procedural codes according to organization specified procedures and assigns ICD-9, CPT and HCPCS codes as appropriate. Provides codes to various departments upon request.
Enters and validates charges using appropriate tools and validates diagnoses with the medical documentation provided.
Compares charges on accounts with the procedures coded and identifies any discrepancies. Notifies Coding Manager of any discrepancies? and collaborates as needed to rectify the account.
Participates in special projects and/or completes other duties as assigned.
Provides coding education to physicians and staff for proper documentation, coding, and charge entry as it pertains to the coding department.
Reviews E&M services and provides feedback to the providers regarding the outcome of the review.
Knowledge / Skills / Abilities:
General understanding health insurance and medical costs, including coding.
Must be able to able to communicate effectively.
Must have proficient computer skills, with the ability to learn applicable internal systems.
Possess a strong work ethic and a high level of professionalism.
Certification of one of the following needed: American Health Information Management Association (AHIMA: Registered Health Information Management Technician (RHIT), Registered Health Information Management Administrator (RHIA), Certified Coding Specialist (CCS), or Certified Coding Specialist Physician Based (CCS-P) OR American Association of Procedural Coders: Certified Professional Coder-Hospital (CPC-H) or Certified Professional Coder (CPC)
Must have a high school diploma or equivalent. Associate?s degree in related field preferred.
Completion of college level courses in medical terminology, anatomy and physiology, disease processes and pharmacology highly desirable.
Completion of ICD-9 or CPT coding course required.
2-5 years coding experience..
