Due to the success of our Brooklyn practice offered through Freelancers Medical, we are looking to hire another full-time Registered Nurse to play the role of Nurse Innovator.
Freelancers Insurance Company (FIC), Collective Primary Care (CPC), and Iora Health are partnering to revolutionize primary care through our unique care model. This model is outside of the traditional fee-for-service payment system, which frees medical providers and other caregivers to spend more time building stronger relationships with patients. Patients will enjoy easy appointment scheduling, access to their doctor and health coach by phone and email, and free wellness groups (such as yoga and meditation). Providers will enjoy the benefits of smaller panel sizes, closer relationships with patients, no billing or coding, and the opportunity to lead systemic change in health care delivery while working with a true team At the heart of our new staffing model is a team of health coaches, who work with patients to coordinate services, unlock personal motivation, and make healthy lifestyle changes. This model of care delivery dramatically improves the patient care experience as well as the quality of clinical outcomes.

Essential Duties And Responsibilities
The Collective Primary Care Nurse Innovator is a key leader within the practice who works closely with the Medical Director to ensure that we develop a high performing team capable of delivering exceptional care and a world-class experience to our patients. Specific tasks will include:
Train, supervise, and lead a team of Health Coaches, Social Workers and Administrative Staff in supporting patients through their clinical and social needs.
Provide leadership for staff members in the form of ongoing training, performance feedback, goal setting, and problem resolution; cultivate a culture of innovation and creativity in service to better patient care.
Actively manage practice activity to ensure smooth patient flow, timely care, and outstanding service to patients.
Ensure that the physical practice is properly equipped with the necessary clinical tools, supplies, and resources.
Triage patients who reach out to the practice via walk-ins, emails, or phone calls, ensuring timely and appropriate care.
Participate in direct clinical assessment and treatment of patients.
We are looking for a licensed Registered Nurse, who has or can obtain a New York state license. At least five years of nursing / professional experience is preferred, preferably in a dynamic setting such as an ER or L&D. The ideal candidate will be a systems thinker, technologically savvy, personable, be able to think out of the box, and someone who values customer service and efficiency highly. We want someone who is idealistic, thoughtful, articulate, hard working, creative, flexible, and able to deal with uncertainty. Candidates with work experience outside of medicine, and those with other advanced degrees or specialization are encouraged to apply.
