Essential Duties and Responsibilities:
The Nurse Innovator is a key leader who partners with the Medical Director to run the practice. Specific responsibilities include:
Train, supervise, and lead our unique care team of Health Coaches, Social Workers, and Administrative Staff to support patients? clinical and social needs, both at the practice and in group meetings in the community.
Develop strong team environment through ongoing training, performance feedback, goal setting, and problem resolution.
Cultivate a culture of innovation and creativity in service of better patient care.
Understand the needs of our sponsors (clients) as well as fostering a relationship with our partner organizations.
Actively manage practice activity to ensure smooth patient flow, timely care, and outstanding service to patients.
Ensure that the physical practice is properly equipped with the necessary clinical tools, supplies, and resources.
Support health coaches in helping patients navigate the health care system when secondary or tertiary care is needed.
Triage patients, and provide direct clinical care.
Licensed Registered Nurse, who has or can obtain a Washington state license.
At least five years of nursing / professional experience is preferred.
Comfort working with a geriatric and chronically ill population essential.
Customer service experience and experience facilitating group learning a plus.
Excited to create a new program from the ground up; idealistic, thoughtful, articulate, hard working, creative, flexible, and able to deal with uncertainty.
