Due to the success of our Brooklyn practice offered through Freelancers Medical, we are looking to hire another full-time Nurse Practitioner (NP).
Freelancers Insurance Company (FIC), Collective Primary Care (CPC), and Iora Health are partnering to revolutionize primary care through our unique care model. This model is outside of the traditional fee-for-service payment system, which frees medical providers and other caregivers to spend more time building stronger relationships with patients. Patients will enjoy easy appointment scheduling, access to their doctor and health coach by phone and email, and free wellness groups (such as yoga and meditation). Providers will enjoy the benefits of smaller panel sizes, closer relationships with patients, no billing or coding, and the opportunity to lead systemic change in health care delivery while working with a true team At the heart of our new staffing model is a team of health coaches, who work with patients to coordinate services, unlock personal motivation, and make healthy lifestyle changes. This model of care delivery dramatically improves the patient care experience as well as the quality of clinical outcomes.

Iora Health is a rapidly growing primary care management services organization that is opening such innovative primary care practices nationwide in an effort to change care delivery as we know it today.
Essential Duties and Responsibilities
The Collective Primary Care NP will work with the Medical Director to play a critical role in both care delivery as well as the ongoing design. Specific tasks will include:
Delivering adult medicine and pediatric services within the practices, through both traditional face-to-face visits as well as non-visit based interactions (e.g. phone, email, groups, home visits, video chat).
Collaborating with a team of health coaches, who, based on a community health worker / promotora model, will help patients to set self-management goals, improve patient self-efficacy, and support physicians.
Participating in development of robust group visit offerings, including educational classes, integrative yoga and fitness classes, etc.
Taking a population management approach to care by viewing the entire community of patients as recipients, and using technology and team resources to direct care towards those most in need.
Participating in communicating and marketing the practice model to new patients as well as other stakeholders such as the medical community, payers, and the press.
Carrying out other appropriate professional duties as may be necessary, including liaison with outside medical consultants, hospitalists, emergency room providers, etc.
We are looking for someone trained and experienced treating both adults and children who has or can obtain a New York State license. The ideal candidate will be an excellent clinician, a systems thinker, technologically savvy, personable, be able to think out of the box, and someone who values customer service and efficiency highly. We want someone who is idealistic, thoughtful, articulate, hard working, creative, flexible, and able to deal with uncertainty. Candidates with work experience outside of medicine, and those with additional advanced degrees or specialization are encouraged to apply.
