THIS JOB HAS EXPIRED Cogent HMG is seeking a Patient Care Liaison for our Program at Williamson County Medical Center in Franklin, TN.
The Patient Care Liaison is responsible for assisting the physicians and Clinical Care Coordinators with facilitating proper continuity of care of all patients admitted and discharged. Responsibilities include but are not limited to:
Essential Functions / ADMISSION PROCESS
Participate in morning assignment meetings
Greet newly admitted patients and introduce Cogent and the hospitalist program. Set expectation for hospital stay and potential discharge date per input from Cogent Hospitalist and Clinical Care Coordinators.
Give each Cogent patient a New Patient Care Kit with marketing collateral materials.
Initiate Discharge Template / Check list
Collect / Validate E2 data elements and deliver printable intake form (aka face sheet) to AC
Verify patient?s PCP information and phone numbers.
Assist AC in notifying PCP of their patient?s admission.
Essential Functions / ROUNDING PROCESS
Make follow up appointments for patients with PCP?s as appropriate and as directed by CCC or physician.
Confirm PCP?s identification and communicate F/U appointments through written charting documentation.
F/U on care delays and on any tests, procedures, consults not started/completed
F/U on unanswered queries and notify Cogent physicians
Assist CCC with facilitation and coordination of patient care as needed.
Essential Functions / SUPPORT ACTIVITIES
Complete Advanced Directive part of PQRI
Review and complete assigned work queues
Enter data into E2 and CCC DPLs as applicable (Follow-up phone calls; landmark events; care delays)
Assist CCCs with QEP audits re: readmissions, patient satisfaction, engagement, assisting physicians with Core Measure Queries.
Assist CCC with facilitation and coordination of post-patient care paperwork, as needed (FEMLA; Homehealth; DME, etc.)
Assist Administrative Coordinator and Program Administrator as needed with VOI when time permits.
Prepare reports and charts for group review and/or posting in Cogent Meeting room
Education, Training and Experience
High school diploma or equivalent and graduation from an accredited American Association of Medical Assistants (AAMA) program; Licensed Medical Assistant (LMA) Preferred.
3+ years experience as a medical assistant or unit clerk/secretary in a hospital setting
Pursuing a degree in healthcare related field a plus.
Medical terminology knowledge, medical records review & management experience, basic computer skills with comfort in navigating between computer programs, excellent telephone etiquette, attention to detail and excellent communications & interpersonal skills )written and verbal) ? ALL REQUIRED
