Cogent HMG is seeking Hospitalist physicians to join our team at Williamson Medical Center.

Williamson Medical Center is a 185-bed hospital that provides comprehensive inpatient and outpatient care, including emergency services, which holds the highest rating possible from the Tennessee Department of Health. Additionally, Williamson Medical Center physicians are credentialed in 53 specialties and sub-specialties. Williamson Medical Center?s dedication to quality and standards of excellence has earned the hospital full accreditation by The Joint Commission.
Williamson Medical Center is located in Franklin, TN just a few short miles outside the progressive city of Nashville. Franklin is a quaint town whose culture is based around its multitude of historical avenues and abundance of surrounding amenities.
Cogent HMG has teamed with Williamson Medical Center to bring premier hospitalist services to the great city of Franklin. The hospitalist team admits both medical and surgical patients 16 year of age or older, providing on-site service 24 hours a day, 7 days a week. Cogent HMG proudly offers an exceptional work-life balance, with a no-call environment; our physicians work a schedule of 7 on and 7 off, 12 hour shifts. Additionally, the hospitalist team utilizes a state-of-the-art electronic billing system.

The city of Franklin was established in 1799, and has grown from a tiny, agricultural community into a strong blend of residential, commercial and corporate citizens. There is an abundance to see and do in Franklin, from a Civil War battlefield, to a host of museums and antebellum mansions, to the thriving downtown shopping area and the Cool Springs Galleria. Just a few miles from town is the Natchez Trace Parkway, and if you like antiques, Franklin has some of the best shops in Tennessee. Essential Functions & Responsibilities:
Admitting medical and surgical patients 16 years of age and older (Adult), but excluding obstetrical
Providing internal medicine or critical care services (to the extent Hospitalist is privileged) to patients
Providing medical coordination and inpatient utilization management for surgical patients
Coordinating with patient?s PCP, hospital, or health plan whichever is applicable, all ancillary services related to inpatient episode
Coordinating with patient?s PCP, hospital, or health plan, whichever is applicable, the discharge planning needs with the patient, patient?s family and the care manager/discharge planner
Providing appropriate communication to PCP in order to insure continuity of care and appropriate follow-up care, including preparation of the discharge summary
Coordinating the transfer of patients to other hospital as required and medically appropriate
Responsible for responding in timely manner to a contact from Cogent HMG Call Center, hospital, regarding the care of patient under care of Hospitalist program
Responsible for the clinical/administrative oversight of the Clinical Care Coordinator
Education, Training & Experience Required:
MD/DO, BC or BE in Internal Medicine or Family Practice (Critical Care or Pulmonary Certification a plus)
Excellent analytical and problem solving skills
Demonstrated persuasive clinical leadership
Excellent communicator, both written and verbal
Ability to work as member of team
Fundamental knowledge of PC based applications
