Dartmouth Health Connect is an adult primary care practice that has been created for Dartmouth College employees and their dependents who receive health insurance through a Dartmouth College-sponsored health plan. Dartmouth Health Connect is an innovative practice that incorporates new models of patient care, staffing, payment and technology. Our model puts patients first, freeing providers from the constraints of traditional fee-for-service billing, coding and payment and insufficient staffing. Dartmouth Health Connect patients enjoy the benefits of improved access to care, enhanced by non-office based encounters (e.g. phone and email), an accessible and transparent medical record, and robust educational offerings. At the heart of our staffing model is a team of health coaches, who (in collaboration with physicians) help patients coordinate services, unlock personal motivation, and make healthy lifestyle changes. This care model and philosophy of care delivery has shown to dramatically improve the patient experience as well as the quality of clinical outcomes.

Dartmouth Health Connect is owned and operated by Iora Health, a rapidly growing primary care management services organization that is opening innovative primary care practices nation-wide in an effort to change care delivery as we know it today.
Dartmouth Health Connect is looking for a full time physician (trained and board certified/eligible in internal medicine or family practice) to help lead our practice and deliver exceptional care to our patients.
Essential Duties And Responsibilities
The Dartmouth Health Connect physician will be an integral member of the Dartmouth Health Connect team. (S)he will play a critical role in care delivery and will work closely with the Medical Director on ongoing design and leadership of the practice. Reporting to the Dartmouth Health Connect Medical Director, this individual will be asked to:
Deliver professional services within the practice, including both traditional face-to-face visits as well as non-visit based interactions (eg. phone and email).
Participate in the ongoing training of a team of health coaches, who, based on a community health worker / promotora model, help patients set self-management goals, improve self-efficacy, and support physicians.
Participate in the evolution of our model by contributing to quality improvement activities at the practice level.
Collaborate with colleagues across the Iora network to ensure ongoing learning and improvement of the practice.
Carry out other appropriate professional duties as may be necessary, including serving as a liaison with outside medical consultants, hospitalists, emergency room providers, etc.
Must be a goal-oriented, high energy, passionate leader who models organizational values and is able to set the tone for a positive work culture.
Must demonstrate outstanding clinical aptitude, using sound judgment in caring for patient needs.
Must demonstrate excellent interpersonal communication skills with a variety of audiences via telephone and electronic means including exceptional listening skills, ability to use appropriate language and demonstrated writing skills.
Must demonstrate ability to think outside of standard operational structure.
Must highly value customer service and efficiency.
Must be a thoughtful systems thinker and technology savvy.
Must be creative, flexible and comfortable with ambiguity.
Must be trained and board certified/eligible in internal medicine or family practice and have or obtain a New Hampshire medical license.
While we would prefer 4+ years of experience, recent residency graduates are encouraged to apply.
Knowledge of the Hanover, NH health care community is preferred.
